The connective tissue stains are more convincing than the H&E but I would confirm with genetics.
I agree it looks like excess blue collagen to my inexperienced eye. Do you have any Masson sections of normal right ventricle in c. 31 year old? Thanks for the case.
I think the macro shows somewhat ‘blobby’ areas of fat arranged around the transverse sections. This, and the patchy fibrosis (with focal lymphoid cells) suggests ARVC is top of my list. I suspect the FH is potentially hiding other cases, particularly if no histology performed. I would advise genetics focused on ARVC and family to go to local inherited cardiac disease clinic review.
