this looks unusual to me. the leakage of RBCs into interstitium means that myocytes may appear stretched/attenuated, but I see no degeneration/ intrinsic pathology (eg myocarditis). the history suggests other organs potentially with same issues. I think this is pointing to an severe endothelial lesion with capillary leak and the changes seen. it is not part of the usual SLE cluster (Libman Sacks, congenital heart block, pericarditis, local thrombosis)…..and of course drug complications of various therapies.
